Finger metacarpophalangeal (MCP) joint collateral ligament sprains should not be overtreated. First-degree sprains may require a brief period of protection, usually consisting of buddy taping for 2-3 weeks. Second-degree sprains are immobilized in mid flexion for 3 weeks. Finger MCP joint hyperextension injuries may be treated by gently flexing the proximal phalanx and immobilizing the MCP joint in 30° of flexion for 2-3 weeks. A dorsal extension-block splint protects the healing volar plate while allowing active flexion of the finger. Early protected motion minimizes postinjury stiffness. Thumb MCP joint hyperextension injuries ("locked MCP joint") are immobilized in 20° MCP joint flexion for 3 weeks.
Every year, thousands of conventional total shoulder replacements are successfully done in the United States for patients with shoulder arthritis. This type of surgery, however, is not as beneficial for patients with large rotator cuff tears who have developed a complex type of shoulder arthritis called "cuff tear arthropathy." For these patients, conventional total shoulder replacement may result in pain and limited motion, and reverse total shoulder replacement is a better option.
Posterior dislocations with associated fractures, also known as complex posterior dislocations, often require open reduction and fixation (ORIF). These dislocations are often associated with significant ligamentous injury. In some cases, complex posterior elbow dislocations may be managed with closed reduction. Posterior elbow dislocations that are neglected, as is not uncommon in developing countries, can often be effectively treated with open reduction. [9] Delayed vascular compromise is an important complication after reduction. All patients should be observed for a period of approximately 2-3 hours after reduction. If no evidence of vascular compromise arises, patients can be sent home with appropriate follow-up and instructions to watch for further problems.

Carpal tunnel release is a surgery used to treat and potentially heal the painful condition known as carpal tunnel syndrome. Doctors used to think that carpal tunnel syndrome was caused by an overuse injury or a repetitive motion performed by the wrist or hand, often at work. They now know that it’s most likely a congenital predisposition (something that runs in families) – some people simply have smaller carpal tunnels than others. Carpal tunnel syndrome can also be caused by injury, such as a sprain or fracture, or repetitive use of a vibrating tool. It's also been linked to pregnancy, diabetes, thyroid disease, and rheumatoid arthritis.
The biceps muscle is located in the front of your upper arm. It is attached to the bones of the shoulder and elbow by tendons — strong cords of fibrous tissue that attach muscles to bones. Tears of the biceps tendon at the elbow are uncommon. They are most often caused by a sudden injury and tend to result in greater arm weakness than injuries to the biceps tendon at the shoulder. Once torn, the biceps tendon at the elbow will not grow back to the bone and heal. Other arm muscles make it possible to bend the elbow fairly well without the biceps tendon. However, they cannot fulfill all the functions of the elbow, especially the motion of rotating the forearm from palm down to palm up. This motion is called supination. To return arm strength to near normal levels, surgery to repair the torn tendon is usually recommended. However, nonsurgical treatment is a reasonable option for patients who may not require full arm function.
A reconstructive transplant, also called a composite tissue transplant or composite tissue allograft, is an operation that involves transplantation of bone, tissue, muscle and blood vessels. A reconstructive hand transplant is an operation tailored to each patient’s individual needs, type of injury and anatomy. This transplants an upper extremity, usually at the level of the forearm and wrist, but sometimes above the elbow, to help restore function after the loss of a hand or arm.

In nonsurgical treatment, progressive physical therapy and rehabilitation can restore the knee to a condition close to its pre-injury state and educate the patient on how to prevent instability.37, 38 This may be supplemented with the use of a hinged knee brace. However, many people who choose not to have surgery may experience secondary injury to the knee due to repetitive instability episodes. Surgical treatment is usually advised in dealing with combined injuries (ACL tears in combination with other injuries in the knee). Depending on the fracture, the bone fragments may be fixed using screws, a plate and screws, or different wiring techniques. Because there is such a wide range of injuries, there is also a wide range of people's specific recovery time for ankle fracture surgery. It takes at least 6 weeks for the broken bones to heal.
TAA is performed either under general anesthetic or nerve block. A tourniquet is used at the time of surgery to control bleeding and improve visualization during the surgery. The ankle is approached from the front or the side depending on the type of implant being used. Bone is then cut, allowing for placement of the metal and plastic components that re-create the ankle joint. Sometimes the patient will have a tight calf muscle or tight Achilles tendon that needs to be lengthened to improve range of motion of the ankle. The wounds are then closed using stitches or staples, and a splint is applied. A period of non-weightbearing in either a cast or cast boot is necessary to allow the implants to heal in place.
The definition of DDH is not universally agreed upon. Typically, the term DDH is used in referring to patients who are born with dislocation or instability of the hip, which may then result in hip dysplasia. More broadly, DDH may be defined simply as abnormal growth of the hip. Abnormal development of the hip includes the osseous structures, such as the acetabulum and the proximal femur, as well as the labrum, capsule, and other soft tissues. This condition may occur at any time, from conception to skeletal maturity. The author prefers to use the term hip dysplasia, considering it both simpler and more accurate. Internationally, this disorder is still referred to as congenital dislocation of the hip.

A broken bone must be carefully stabilized and supported until it is strong enough to handle the body's weight and movement. Until the last century, physicians relied on casts and splints to support and stabilize the bone from outside the body. The advent of sterile surgical procedures reduced the risk of infection, allowing doctors to internally set and stabilize fractured bones. During a surgical procedure to set a fracture, the bone fragments are first repositioned (reduced) into their normal alignment. They are held together with special implants, such as plates, screws, nails and wires. Internal fixation allows shorter hospital stays, enables patients to return to function earlier, and reduces the incidence of nonunion (improper healing) and malunion (healing in improper position) of broken bones. The implants used for internal fixation are made from stainless steel and titanium, which are durable and strong. If a joint is to be replaced, rather than fixed, these implants can also be made of cobalt and chrome. Implants are compatible with the body and rarely cause an allergic reaction.
Superior capsule reconstruction (SCR) is a promising alternative treatment for irreparable posterosuperior rotator cuff tears (Figure 1). It utilizes a graft from the superior glenoid to the greater tuberosity to stabilize the humeral head. In a study by Mihata and colleagues of 23 patients who underwent SCR with a fascia lata autograft at a minimum of 2 years follow-up, the American Shoulder and Elbow Surgeons (ASES) score improved significantly from 23.5 preoperatively to 92.9. Postoperative MRI showed 83% of patients had intact reconstructions with no progression of muscle atrophy.
